The form is pretty straightforward, but to help you along here are some extra instructions:

• Participant Information: All fields are required.
• Medical Conditions: Please note any that we should be aware of.
• Emergency Contact: Required fields.
• Program Name: $20/year for an individual annual membership, $35/year for a household. We encourage additional donations to the club if you are able. This helps us with special events and promotional items.
• Volunteering: Please check any of the boxes that interest you.
• Waiver: Please read and sign and date.
• Photos: If you do *not* want your photo used, then initial.

Print, fill out, and sign the form, and send it and your check to:

That’s it! You should get an email from an Upper Valley Running Club officer within a week or so.
